  • Barking, Havering and Redbridge Maternity Referrals - Congratulations on your pregnancy! Please complete all relevant details as soon as possible following your positive pregnancy test so we can book you in for your first appointment in a timely manner. Any missing information may slow this process. You will normally be seen for your first appointment between 8 to 10 weeks, depending on your gestation when we receive your self-referral. We advise that all pregnant women take 400 micrograms of Folic Acid until they are 12 weeks pregnant and 10 micrograms of Vitamin D a day for the duration of their pregnancy.   • Hull Maternity Services SPA - Congratulations on your Pregnancy. Hull Women & Children’s Hospital provide high quality, inclusive care for pregnant women and their families in Hull and the East Riding of Yorkshire. If you would like to book for your antenatal care with us please complete our online referral below. If you haven't started already we suggest that you start taking Folic Acid, if your BMI is under 30 you need to take the standard 400mcg dose which is available over the counter or in pregnancy vitamins, if your BMI is 30 or above you will need to contact your GP to obtain a 5mg dose of Folic Acid, you will need to take this until you are 12 weeks pregnant. We also suggest everyone takes a 10mcg dose of Vitamin D daily for the duration of your pregnancy, this is available over the counter or in pregnancy vitamins. Should you have any problems completing this referral on line please contact our Community Admin Team on 01482 382658 who will be happy to help you. Hull University Teaching Hospitals offers you the choice of where you would like to have your baby, these include The Alongside Midwifery Led Unit (AMLU - Fatima Allen Birthing Centre), the labour ward within the hospital or your home (Homebirth). All of these options will be discussed in detail at your booking appointment. In the meantime if you experience any pain or bleeding and are worried about your pregnancy before 16 weeks, please contact your GP/Walk in Centre or 111 straight away. You do not need to inform your GP that you are pregnant however if you do have any existing health problems, it’s a good idea to make an appointment to see your GP, so that they can review any medication you’re taking and see if you need any additional care. You can also use this to refer a pregnancy on behalf of someone else.
  • Oxford University Hospitals Pregnancy Self-Referral (SPA) - Congratulations on your pregnancy and thank you for choosing Oxford University Hospitals - we look forward to supporting you!   • Harrogate Referral Location - If you are pregnant and are considering giving birth at Harrogate, please get in touch as soon as possible so that we can start planning your care together. You may also complete this from at any stage in pregnancy if you wish to transfer your care to Harrogate from another maternity care provider. You can also complete this form to refer a pregnancy on behalf of a woman. Our aim is make sure you have access to the right care from day one. Harrogate will triage your referral based on the information you provide and the hospital will contact you in the next few weeks to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away. If you have a history of: Ectopic or molar pregnancy, 3 recurrent miscarriage, histoty of a late miscarriage or pregnant with an intrauterine device in place still please incicate in the free text of the medical history.   • Northamptonshire Pregnancy Referrals - Northampton General Hospital have introduced a new way for patients to refer themselves into maternity care when they become pregnant. The new system will help ensure newly pregnant patients get the right care and support from the very start of their pregnancy journey. After you have filled in the form you will receive a letter with a date for your first scan at between 12 and 14 weeks of pregnancy and a midwife will be in touch to arrange your first appointment with our team.

  • Somerset SPA - If you are more than 6 weeks pregnant you can use this maternity referral form to tell us about your pregnancy.   • Dorset Referrals - You can refer yourself to Dorset maternity services by completing this self-referral form. If you don’t wish to have your baby at a Dorset hospital, please directly contact the hospital you wish to birth at. You may still receive some of your care from Dorset midwives. If you are considering a homebirth, please complete the self-referral form. You will then receive a booking appointment, which will happen when you’re around 8 - 12 weeks pregnant, or as soon as possible if you believe that your pregnancy is further along. There is no need to see your GP unless you have existing health problems. If you do have any existing health problems, it’s a good idea to see your GP when you find out that you’re pregnant, so that they can review any medication you’re taking and see if you need any additional care.
